You will receive a response stating that we received your request; we will send you a second email with your return authorizatin number within 2 business days of receiving your email. 

This form must be completed within 5 days of receiving your order, or your return authorization request will be rejected.  You must then mail your return in within 5 days of receiving your return authorization number.
